Have you and your child filled out the state required Concussion Form for 2010? Make sure to fill it out and mail it in right away. Please do not fax.

  • • Mon., Tue., Thur. & Fri. 3:45-5:30 p.m. (7-12 years old) & 5:30-7:00 (13-17 years old)
    • Wed. 2:00-4:30p.m. (7-12 years old) & 4:30-7:00 p.m. (13-17 years old)
